Abstract

"Mitoshishi" refers to someone who supports and accompanies the dying in their final moments, whether as a professional or as a support for families during home-based end-of-life care. This book is an essay by Kumiko Shibata, a "mitoshishi" who has been facing life and death for over 25 years. It is said that about 80% of Japanese people spend their final moments in hospitals, but approximately 50% express a desire to die at home. However, in modern society where the culture of home-based end-of-life care has diminished, it can be challenging for caregivers to fulfill this wish due to various circumstances.

With over 200 cases as a basis, the author shares insights on the mindset and practical approach to end-of-life care, as well as the importance of utilizing "ending notes" and the significance of inheriting the soul of the departing individual. The book also includes a dialogue with Dr. Minoru Kamada, Honorary Director of Suwa Chuo Hospital, where they discuss the birth story of the "mitoshishi" profession, their thoughts on death, and their shared dream of community-based integrated care.
