This new tool helps reduce mistakes and relieve frustration by simply answering the following two questions:
Question 1: How many people were involved in the task?
Question 2: How often is the task performed?

By asking these two questions, you can determine which of four areas the failure falls into.
It visualizes the types of failures that are likely to occur in each area. It helps you understand the reasons for failure and develop countermeasures and preventative measures.

Author’s Information
He holds a PhD in Engineering from Stanford University, is a specially appointed researcher at the University of Tokyo, and is the vice president and secretary general of the Society for the Study of Failure. Born in Osaka in 1959. After completing a master's degree at the University of Tokyo Graduate School of Engineering, he joined General Electric's Nuclear Power Generation Division. He then earned a PhD in Mechanical Engineering and Information Engineering at Stanford University and joined Ricoh Corp. In 2000, he founded SYDROSE LP and became its general partner (current position). In 2002, he became vice president of the NPO Japan Society for the Study of Failure.
